I experienced this exact problem today.
My mistake was placing the bootstrap modal html within an existing form element.
Chrome, Firefox automatically removes
form tags within an existing form element.
Moving your modal window to the bottom of the body tag may help.
Solutions is
Count the number of form tags opened and closed.
The form closed properly or not please check it..
